The Doppler assessed diastolic dysfunction is shown by the vertically hatched middle circle and the pure diastolic dysfunction excludes the horizontally hatched inner circle. The Doppler patterns have different implications in pure diastolic dysfunction and systolic dysfunction. Note that the inner circle of systolic dysfunction has elements of diastolic dysfunction too. But in clinical situations it is better to view this part as 'systolic dysfunction' for treatment purposes. Any cardiac pathology has the potential to produce 'diastolic dysfunction' hence in clinical practice the use of the term is meaningless.
